Enrollment
For students in grades K-12, whose parents/legal guardians reside in the Southeastern Local School District:
Please follow the steps. During your enrollment appointment, the parent/guardian must be able to prove residency (see item 2e)
1) Schedule an Enrollment Appointment: Call the appropriate school office, Elementary or Middle/High, (740-774-2003) to schedule an Enrollment Appointment.
2) Bring the following documents to your Enrollment appointment:
a. Child's Birth Certificate
b. Child's Immunization records
c. Parent's photo ID
d. Custody documents, if applicable. Parent assigned as "residential" parent in the court document must reside in the Southeastern Local School District .
e. Proof of residency:--parent/guardian and student must reside in the Southeastern Local School District
■ If you own or rent a home in the district, please complete the Residency Verification Form and be sure to bring one item from Category A and one item from Category B as indicated on the form.
■ If you are not the homeowner, renter, lessee, or tenant of the property, you must complete the Friend and Family Packet and provide all documentation indicated in the form. Parent/guardian must complete page 1 and the homeowner/lessee must complete page 2, including a notarized signature.
■ If homeless and eligible to enroll under McKinney-Vento Act, please inform us of your situation during your enrollment appointment
